Download your COVID-19 vaccine records and test results by April 30

All current and former campus community members who participated in the University of Arizona COVID-19 testing and/or vaccine upload programs were sent an email this week with the following message.


You have until April 30 to view and download your COVID-19 vaccination records and Test All Test Smart test results. After this date, your records will no longer be accessible as part of the phaseout of the University's Test, Trace, Treat program. This is your opportunity to retain a copy for your records before they are archived.

Follow these steps to retain a copy of your COVID-19 vaccination records:

  1. Log in to vaccine.wellcheck.arizona.edu using your NetID and password.
  2. Select "Download Vaccine Documents" to save a copy of your vaccination document.

Note:

  • If you received your vaccination at Campus Health or uploaded your records to Campus Health, your Campus Health COVID-19 records will remain available on PatientLink as part of your medical records.
  • If you are a University employee and your COVID-19 vaccination was verified through the Arizona State Immunization Information System without a vaccination document, this message will appear on the webpage: "You opted to verify your vaccination status without uploading your vaccine document."
  • If you did not upload your COVID-19 vaccination document to the University system and you obtained your COVID-19 vaccination in Arizona, visit myIRmobile.com to obtain a copy of your records.

COVID-19 test results, including CATS TakeAway and on-site testing

  1. Log in to results.wellcheck.arizona.edu using your NetID and password.
  2. Select the test result date to view the result. If you have multiple test results, select "Expand All" to view the details of all results.
  3. Take a screenshot of your results and/or download the test result letters. Individual test results must be downloaded one at a time.

Note: If your COVID-19 test results are in Campus Health records, they will remain available on PatientLink as part of your medical records.

Archiving COVID-19 Data

After April 30, your COVID-19 testing and vaccination records will be archived and stored using the University's strongest security and monitoring efforts. All records are fully encrypted, and access to the HIPAA-compliant storage system by authorized University users is documented. Additionally, the system undergoes regular risk assessments to ensure continued attention to appropriate security controls. Data will be archived following the University and Arizona Board of Regents retention schedules. Your data privacy and security are of utmost importance.
